My headphone volume seems to vary during a track so that louder parts are made quieter and quieter parts are made louder. This is a pain particularly at start and end of tracks where they fade in and out. Is there a way to stop this or is this a bug?

It was set to 1.0. Changed it to 0.5 and now appears to be OK.
Does this mean with 1.0 it was probably clipping the volume?

Yes, at 1 it will boost the volume, probably above clipping level.
